Normally they are worn on the 2nd toe as this is the most comfortable. They do not fall off as you have a pad under your toe that holds it in place. You can shower, wear shoes, go to the gym and they stay in place.

History of the Toe Ring.
Toe rings have been around for hundreds of years. Adorning toes over the centuries.
In India even to this day at certain Hindu wedding ceremony's the bridegroom will place a pair of matching toe rings on the brides toes. She will wear these for life. At any festival you will find traditional Indian woman adorned with jewellery from their nose ring to their toes. Spectacular.
In Western countries Eastern Europe, Greece, Spain, Northern Italy, Western Europe, England, Wales, Scotland and Ireland the Celtic people which go back 25 centuries the woman were known to wear belt chains around their waists sometimes with bells at the end, bangles, necklaces, rings and often toe rings.
The Tuareg are West African people. They are semi-nomadic people of Niger, Mali, Algeria, Burkina Faso, and Libya also known as the blue people of the Sahara. Called this because of their indigo clothing which sometimes stains their skin. Tuareg crafts consists mainly of metal working silver jewellery.
